On October 18th and 19th, the Ontario Community Support Association will be hosting Ontario’s largest and most representative conference dedicated to home and community care. This years theme, “From Roadmap to Reality: Navigating Change Together” will explore the challenges and opportunities of health system transformation for providers, clients and communities.
We are excited to announce that the Hamilton Seniors Isolation Impact Plan (HSIIP) will be presenting at the conference on October 18, 2017. April Morganti, Executive Director of AbleLiving/Thrive Group and Amanda Bradford-Janke, Research Manager at the Gilbrea Centre will be running a workshop entitled “Seniors Isolation Coalition: Using Connectors and Collaboration for Community Wide Impact”. This workshop will highlight HSIIP and how the project aims to reduce social isolation among seniors in the Hamilton area by linking seniors with existing services and social opportunities in the community.
